The ending of Infirmary deliberately leaves much of its supernatural mystery unresolved, particularly the dark mind-transfer experiments and the possibility of black magic taking place inside the building. After the power fails, Edward and building administrator Ms. Downey (or a phantom resembling her) descend into the unlit basement, where the security cameras cannot reach. The door locks behind them, leaving their fates unclear. When police officers later investigate, they find the bodies of Lester (the veteran security guard), and Ms. Downey. Edward, however, is nowhere to be found, and investigators discover that “Edward Yoon” is an alias, making him a prime person of interest in the apparent murders.

Who Survives at the End of Infirmary?
The film’s opening preface suggests that two bodies were discovered after the events captured in the found footage, which initially leads viewers to believe the victims will be the two security guards. Ms. Downey’s unsettling appearances also make her seem like a possible culprit. However, the final revelations turn those expectations around. Lester and Ms. Downey are the bodies found by police, while Edward is the one who survives and disappears. Additional unseen footage shows him sitting silently, but the closing text and epilogue confirms who seemingly survived that night.
